The 3 forms available for download here are the player registration form, the player code of conduct form, and the internet website authorization form. Please download and carefully read all 3 forms. The registration form and code of conduct form are mandatory forms which must be completed by the player and guardians, and sent or given to the Sports For Special Kids organization before a player can participate in our activities. The internet authorization form is not mandatory for a player to participate in our activities, but must be completed and received by us if the player wishes their picture to be included in one of our internet website picture galleries.

The forms are in Adobe Reader PDF format, and you must have Adobe Reader installed on your system to access these files. Adobe Reader is a free program and free download, and is widely used on the internet these days, we recommend that if you currently do not have Adobe Reader installed, to please take this time to download and install it now directly from Adobe Systems:
